Seeking a compassionate and dependable Case Manager to support clients in a permanent supportive housing program. Responsibilities include crisis intervention, advocacy, safety planning, conflict resolution, group facilitation, and helping clients access housing, income, and community resources. The role also involves maintaining accurate client records, supporting daily living activities, and promoting a safe, respectful environment.
Candidates should have experience with trauma-informed care, crisis intervention, and case management, along with strong communication and computer skills. A Bachelor’s degree in Social Work or a related field is preferred, though relevant experience may substitute. Must have a valid driver’s license, pass a background check, and be comfortable working flexible hours, including evenings or weekends as needed.
Preferred qualifications include experience with mental health, substance abuse, aging populations, and individuals with disabilities.
